Red Skull plays a surprising part in Avengers: Infinity War and Endgame as the stonekeeper of the Soul Stone. In the movie, it's revealed he was transported to Vormir after handling the Tesseract and now guides people in its use. He has an ethereal appearance and is just a head in a floating cloak.
According to one of the skilled concept artists working on the film, he was supposed to look very different
Jerad S. Marantz is a professional concept artist who's worked on major motion pictures like Guardians of the Galaxy (2014), Dawn of the Planet of the Apes (2014), and Thor: The Dark World (2013).

He said on Instagram "At this stage in development, they wanted to explore versions of the character that showed how the Soul Stone twisted and deformed Red Skull.'
